Job Responsibilities:
- Have extended knowledge of care plans development & implementation
- Fluent skills and completion MDS 3.0
- Must also possess the ability to plan, organize, develop, implement, and interpret the programs, goals and objectives that are necessary for providing quality care.

Requirements:

Knowledge / Experience:
- Must have 2-3 YEARS SNF/LTC EXPERIENCE
- Must have MDS / TILE experience
- Must be fluent in MDS 3.0
Licensure / Certifications:
- Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in this state.
